Health and wellness is an essential component to a child’s education. Below you will find information needed for the school year. If your child’s medical condition changes or your child develops a new medical condition, please inform both your child’s teacher and the nurse.

 

MEDICATION

  • If your child will need medication during the school day, the Medication Authorization form must be completed by both a healthcare provider and the parent/guardian. This form is needed for all medication, including over the counter medications
  • Emergency medications, such as inhalers and EpiPens, can be kept with students in certain situations however, the Medication Authorization form would still need to be completed. All other medication must be kept in the school office. 
  • Over the counter medication must be in the original packaging. Prescription medications must be in the original packaging labeled with the child’s name from the pharmacy.

 

ASTHMA

  • All students with an asthma diagnosis requiring medication will need to have an Asthma Action Plan on file at school. The action plan found on this page can be used or your healthcare provider’s preferred action plan can be turned in. The Medication Authorization form will need to be completed as well for inhalers.

 

FOOD ALLERGIES

  • All students with food allergies that may require medication need to have an Food Allergy Action Plan on file at school. The action plan found on this page can be used or your healthcare provider’s preferred action plan can be turned in. The Medication Authorization form will need to be completed as well for EpiPens and/or Benadryl.

 

VISION AND HEARING SCREENINGS  

  • Annual vision screenings are mandated per IDPH and ISBE for Pre-school, Kindergarten, 2 nd grade, 8th grade, transfer students, and teacher referrals.  
  • Annual hearing screenings are mandated per IDPH and ISBE for Pre-school, Kindergarten, 1st grade, 2nd grade, 3rd grade, transfer students, and teacher referrals.  
  • If the parent/guardian does not want their student screened, please put in writing and send it to the school nurse.
