Welcome to Extended Day! 

Hello, my name is Jessica Mooi, and I am the Extended Day Director at St. Damian  School. Our Extended Day Program is before and after school care for enrolled students.  Our program provides professional care, supervision, recreation, and enrichment  activities. Students have the opportunity to play outdoors with friends, engage in organized  games and crafts, complete their homework, work on projects, and enjoy an afternoon  snack. Our program fosters the physical, mental, emotional, social and religious  development of our students within a Catholic environment. 

For the Morning Session – Doors open at 6:30a.m. for drop off. Entrance to Extended Day is  through Door “F.” You may send in a nutritious breakfast if your child(ren) did not eat at  home. We will be glad to warm up any breakfast in the toaster or microwave. We are a  peanut/nut free environment. Please do NOT send in any food items that contain any type  of nut or are made in a factory where nuts are processed. 

For the Afternoon Session – Students are checked in at 3:15 p.m. Pre-K and Kindergarten  are picked up directly from the classroom and brought directly to Extended Day. All other  grades are sent by their classrooms to Extended Day before all school dismissal. Our  program will provide snacks and water upon arrival in the Extended Day room. 

PM Pick Up - Extended Day is available until 6:00 p.m. Please ring the doorbell or call our  direct line (708) 342-8509 upon arrival to pick up your child(ren). Students are released  through Door “F.” If a student is not picked up by 6:00 p.m., you will receive a phone call  from one of our staff members. If we cannot reach you, we will contact someone on your  emergency list. Service fee after 6:00 p.m. is $1.00 per minute, until the child(ren) are  picked up. Please read the Handbook regarding late pick-up. 

Pre-K 3- & 4-year-old half-day students - The program is offered BEFORE SCHOOL ONLY. 

Toilet Trained – All Pre-K/Kindergarten students must be completely toilet trained and be  able to use the restroom without assistance. If your child is not completely trained or starts  to have consistent toilet issues, they will be asked to leave the program until they are  completely trained. If there is an accident, you will be called and may require someone to  come pick up your child immediately. Please refer to the Handbook regarding toilet training  and accidents.

Registration - To reserve space for your child/children in the Extended Day Program, please  complete all forms found BELOW:  

• Application Form, Emergency Card, Miscellaneous Information Sheet and  Handbook and Billing Signature Form, along with the appropriate registration fee. 

Students are NOT allowed to participate in the program until all forms and fees are turned  in. Any unpaid Extended Day bills from the previous school year, including Summer Camp,  needs to be paid in full in order for student(s) to attend. Please refer to the handbook on  the website for all details regarding the program. 

Registration Fee - $50.00 for 1 child 

 $60.00 for 2 or more children 

Service Fees - $ 8.50/per hour per student 

 $12.00/per hour per family 

Billing - Single students and families will be charged for the 1st hour automatically and will  be billed by the ½ hour after that. Bills are posted to the “Incidental Expenses” category on  your FACTS account weekly. Payment is due 10 days after the invoice is generated. If you  fail to meet the program’s weekly payment by the due date, a late charge of $40.00 is  applied automatically by FACTS Management. Lack of payment could potentially result in  suspension or termination from the program. All payments will be paid directly to your  FACTS account. To pay for the registration fee, please go to the area that says, “Ext. Day  Registration”. For normal weekly payments, go to the area that says, “Ext. Day - Pymt.”. We  have moved exclusively to on-line payments through your FACTS account, the only  exception is paying your registration fee payment by check, which can be made out to “St.  Damian School”. Cash payments will not be accepted for any transaction. Please make  sure you read the Extended Day Handbook for the details of our Program before turning in  all forms.
